18F-2-fluoro-2-deoxy-D-glucose (18F-FDG) is a radiopharmaceutical glucose analog used in positron emission tomography (PET) imaging. Developed and produced locally by Eastern Health in Newfoundland and Labrador, Canada, this specific formulation is intended to improve patient access to diagnostic imaging within the province. The molecule is transported into cells by glucose transporters (GLUTs) and subsequently phosphorylated by hexokinase. Due to the substitution of a hydroxyl group with a fluorine-18 isotope at the C-2 position, the resulting 18F-FDG-6-phosphate cannot be further metabolized via glycolysis and remains trapped within the cell. This metabolic trapping allows for the visualization and quantification of glucose consumption in tissues, making it a vital tool for the diagnosis and staging of various conditions, including malignant tumors, neurological disorders, and inflammatory diseases.
