18F-ER176 is a second-generation radiopharmaceutical PET tracer targeting the 18 kDa translocator protein (TSPO), which is highly expressed in activated microglia and serves as a biomarker for neuroinflammation. Developed to address the limitations of earlier TSPO tracers, 18F-ER176 exhibits high affinity and, crucially, shows minimal sensitivity to the rs6971 genetic polymorphism that affects the binding of many other TSPO ligands. This makes it a valuable tool for imaging neuroinflammatory processes in various central nervous system disorders, including Alzheimer's disease, multiple sclerosis, and traumatic brain injury.
