18F-FCOB04 is a fluorine-18 labeled radiopharmaceutical developed by Peking Union Medical College Hospital for use as a positron emission tomography (PET) imaging agent. It selectively targets monoamine oxidase B (MAO-B), an enzyme primarily located in the outer mitochondrial membrane of astrocytes. In neurodegenerative diseases, MAO-B expression is significantly upregulated due to reactive astrogliosis, a key pathological feature of neuroinflammation. By binding to MAO-B, 18F-FCOB04 allows for the non-invasive visualization and quantification of astrogliosis using PET imaging, serving as a diagnostic tool and biomarker for disease progression in conditions such as Alzheimer's disease and Parkinson's disease. It is currently being evaluated in early-phase clinical trials to assess its safety, biodistribution, and diagnostic utility.
