18F-fluoroboronophenylalanine ([18F]FBPA) is a radiopharmaceutical positron emission tomography (PET) tracer developed primarily as a companion diagnostic for Boron Neutron Capture Therapy (BNCT). It is a fluorine-18 labeled analog of boronophenylalanine (BPA), the boron carrier used in BNCT. The molecule is actively transported into cells via the Large Neutral Amino Acid Transporter 1 (LAT1/SLC7A5), which is frequently overexpressed in malignant tissues such as gliomas, head and neck cancers, and melanomas. By mimicking the pharmacokinetic behavior of the therapeutic agent BPA, [18F]FBPA-PET imaging allows clinicians to visualize and quantify the tumor-to-normal tissue (T/N) ratio of boron accumulation. This information is critical for identifying patients most likely to benefit from BNCT, optimizing treatment planning, and performing accurate dosimetry.
