18F-NOS is an investigational positron emission tomography (PET) radiotracer developed by the University of Pennsylvania for the molecular imaging of inflammation. It specifically targets nitric oxide synthase (NOS), an enzyme that is upregulated during inflammatory processes. In the context of cardiac amyloidosis, particularly light chain (AL) amyloidosis, 18F-NOS is being evaluated to assess the underlying mechanism of cardiotoxicity by imaging myocardial inflammation, which is hypothesized to contribute to disease progression alongside amyloid deposition. As a diagnostic small molecule radiopharmaceutical, it allows for the non-invasive visualization of inflammatory activity in the heart, distinguishing it from tracers like Florbetaben which target the amyloid deposits themselves.
