18F-tetrafluoroborate (18F-TFB) is a positron emission tomography (PET) radiopharmaceutical that functions as a high-affinity substrate for the sodium-iodide symporter (NIS), also known as SLC5A5. This tracer mimics the biodistribution of iodide, being actively transported into cells expressing NIS, such as thyroid follicular cells and salivary glands. It is primarily developed for the diagnostic imaging of differentiated thyroid cancer (DTC), offering superior spatial resolution and sensitivity compared to traditional iodine isotopes like 123I or 131I. Beyond oncology, 18F-TFB is utilized as a reporter probe in gene therapy and cell tracking applications, allowing for the non-invasive monitoring of therapeutic cells (e.g., CAR-NK cells or dendritic cells) that have been engineered to express the human NIS gene.
