18F-TPP (4-[18F]fluorophenyltriphenylphosphonium) is a radiopharmaceutical small molecule developed by Massachusetts General Hospital. It functions as a PET imaging tracer that targets the mitochondrial membrane potential in cardiac tissue. By accumulating in cardiac cells based on the electrochemical gradient across mitochondrial membranes, it allows for the visualization of myocardial viability and electrical activity. This imaging agent is primarily being evaluated in Phase 2 clinical trials to guide catheter ablation procedures in patients with recurrent ventricular arrhythmia, particularly those with scar-mediated ventricular tachycardia following myocardial infarction, by helping clinicians identify the precise sources of arrhythmia.
