Overview

1928zT2 CAR-T cells are a third-generation chimeric antigen receptor T cell therapy engineered to target CD19-positive leukemic cells, incorporating a unique construct with the Toll/interleukin-1 receptor (TIR) domain from Toll-like receptor 2 (TLR2) in addition to typical costimulatory signals (CD28, CD3ζ). The addition of the TLR2 domain enhances T cell activation and effector function, particularly against extramedullary leukemia, which is otherwise challenging for conventional CAR-T cells. The therapy is primarily developed for relapsed or refractory B-cell acute lymphoblastic leukemia (B-ALL) with extramedullary involvement. Clinical trials have demonstrated that 1928zT2 CAR-T cells can induce complete remission in such patients, with manageable safety profiles (notably, cytokine release syndrome but minimal neurotoxicity)[1][7].
