1D9 CAR-T is an experimental second-generation chimeric antigen receptor (CAR) T-cell therapy targeting the purinergic receptor 5 (P2RX5), specifically the full-length ancestral variant (P2RX5-FL). Developed by researchers at the Children's Hospital of Philadelphia and the University of Pennsylvania, this therapy is designed for the genotype-guided treatment of lymphoid and plasma cell malignancies, including B-cell lymphomas, multiple myeloma, and T-cell acute lymphoblastic leukemia. The P2RX5-FL variant is predominantly expressed in individuals of African ancestry, whereas most other populations express truncated, undetectable variants due to specific single nucleotide polymorphisms. The 1D9 CAR-T utilizes a single-chain variable fragment (scFv) derived from the 1D9 mouse monoclonal antibody and has demonstrated potent cytolytic activity in preclinical models against P2RX5-FL-positive cell lines and primary patient samples, including those that have escaped CD19- or BCMA-directed therapies.
