AER101 is a nebulized formulation of **2-deoxy-D-glucose (2-DG)**, a glucose analog that acts as a competitive inhibitor of glycolysis. Developed by the **University of Oklahoma**, AER101 is designed for direct delivery to the lungs via a hand-held nebulizer to target lung tumors, specifically in patients with **non-small cell lung cancer (NSCLC)**. Once inside the cell, 2-DG is phosphorylated by **hexokinase** to 2-deoxy-D-glucose-6-phosphate (2-DG-6-P). Unlike glucose-6-phosphate, 2-DG-6-P cannot be further metabolized by phosphoglucose isomerase and accumulates, competitively inhibiting both hexokinase and phosphoglucose isomerase. This disruption of the glycolytic pathway leads to ATP depletion, cell cycle arrest, and apoptosis in cancer cells, which are often highly dependent on glycolysis (the Warburg effect). AER101 is being evaluated in a pre-operative setting to reduce tumor metabolic activity prior to surgical resection.
