Overview

2-deoxyuridine is a naturally occurring pyrimidine nucleoside and a metabolic intermediate in the DNA synthesis pathway. Clinical research presented at the AHA 2025 meeting has identified elevated circulating levels of 2-deoxyuridine as a significant biomarker for recurrent coronary events, specifically secondary myocardial infarction (MI), in patients with non-ST-segment elevation acute coronary syndrome (NSTE-ACS). Mechanistically, 2-deoxyuridine promotes atherosclerosis and neointimal hyperplasia by stimulating the proliferation and migration of vascular smooth muscle cells (VSMCs) and inducing a loss of contractility. These pro-atherogenic effects are mediated through the activation of the P2Y2 receptor (P2Y2R), as demonstrated by the reversal of these effects using specific P2Y2R antagonists. Consequently, 2-deoxyuridine is being investigated as both a prognostic biomarker and a potential therapeutic target for the prevention of secondary cardiovascular events.
