Drug intelligence / Profile preview
20% hypertonic saline solution is a concentrated electrolyte solution utilized as a hyperosmolar therapy for the management of intracranial hypertension in patients with moderate-to-severe traumatic brain injury (TBI). Investigated primarily by Nantes University Hospital (CHU Nantes) through clinical trials such as COBI (NCT03143751) and COSMOS-TBI (NCT04134195), this therapy involves a continuous intravenous infusion designed to maintain serum sodium levels within a target range of 150-155 mmol/L for at least 48 hours. The mechanism of action relies on the creation of an osmotic gradient between the blood and the brain tissue, which draws excess fluid out of the swollen cerebral parenchyma and into the vasculature, effectively reducing cerebral edema and lowering intracranial pressure (ICP) to prevent secondary brain injury.
