211At-labeled anti-CD3 antibody is an alpha-emitting radioimmunotherapy candidate designed for rapid, targeted, and non-genotoxic T-cell depletion. It consists of an anti-CD3 monoclonal antibody (specifically clone SP34-R1) conjugated to the alpha-particle emitting radioisotope astatine-211 (211At) via a boron cage pendant group (B10). The mechanism of action involves the delivery of high-energy, short-range alpha radiation directly to CD3-expressing T-cells, causing potent cell death while minimizing off-target toxicity and avoiding the genotoxicity associated with conventional chemotherapy-based lymphodepletion. Developed by researchers at the University of Washington and Fred Hutchinson Cancer Center, this agent is being evaluated for applications in CAR T-cell therapy conditioning, HIV cure strategies, and the treatment of T-cell-mediated conditions such as autoimmune diseases and T-cell malignancies.
