**211At-methylene blue** is an experimental, systemically administered targeted alpha-radiotherapy agent comprising methylene blue labeled with the alpha-particle emitter astatine-211. Methylene blue has high affinity for melanin, enabling preferential accumulation in pigmented melanoma cells and localized delivery of short-range alpha radiation that causes lethal cellular damage. In human melanoma xenograft models in nude mice, intravenous 211At-methylene blue inhibited primary tumor growth, lymph-node metastases, and micrometastatic disease in a pigmentation-dependent manner, with greater effects in highly pigmented tumors and limited normal-tissue toxicity. It has been reported only in preclinical academic research and has no identified commercial brand or active commercial development program.
