Overview

212Pb-TCMC-YS5 is a novel CD46-targeted alpha-particle radioimmunotherapy developed for the treatment of metastatic castration-resistant prostate cancer (mCRPC). It consists of the human monoclonal antibody YS5, which binds to a tumor-selective epitope of CD46, conjugated via the TCMC chelator to Lead-212 (212Pb). 212Pb acts as an in vivo generator for the high linear energy transfer (LET) alpha-emitting daughter isotopes 212Bi and 212Po. This targeted radiotherapy delivers potent alpha radiation directly to malignant cells, inducing lethal double-strand DNA breaks regardless of oxygen levels or resistance to conventional chemotherapy. Preclinical studies have demonstrated significant tumor growth inhibition and improved survival in models of both adenocarcinoma and small cell neuroendocrine prostate cancer subtypes.
