213Bi-MX35 is a targeted alpha-radioimmunotherapy (α-RIT) agent composed of the monoclonal antibody MX35 labeled with the alpha-emitting radioisotope Bismuth-213 (213Bi). The MX35 antibody specifically targets the sodium-dependent phosphate transport protein 2b (NaPi2b, also known as SLC34A2), which is overexpressed in over 90% of human epithelial ovarian cancers. By binding to this surface antigen, the conjugate delivers high-linear energy transfer (LET) alpha particles directly to tumor cells and micrometastases. These alpha particles cause dense ionization along their short path (50-80 μm), resulting in irreparable DNA double-strand breaks and cell death while sparing distant healthy tissues. This drug has been primarily investigated for the treatment of minimal residual disease and peritoneal carcinomatosis in ovarian cancer patients following chemotherapy, typically administered via the intraperitoneal route.
