225Ac-Dotadipep is a PSMA-targeted alpha-particle radioligand therapy (TAT) currently in preclinical development by FutureChem for the treatment of prostate cancer. The drug consists of a ligand, Dotadipep, which specifically binds to the prostate-specific membrane antigen (PSMA) overexpressed on prostate cancer cells, conjugated to the alpha-emitting radioisotope actinium-225 (225Ac). Upon binding and internalization, the high-energy alpha particles emitted by 225Ac induce lethal double-strand DNA breaks within the cancer cells, offering a potent therapeutic effect even in cases resistant to beta-emitting radiopharmaceuticals like 177Lu-PSMA-617. The use of alpha emitters is intended to provide higher linear energy transfer (LET) and a shorter range in tissue, potentially increasing efficacy while reducing off-target toxicity.
