225Ac-PD-32766 is a novel radiopharmaceutical consisting of a macrocyclic peptide conjugated to the alpha-emitting radionuclide actinium-225 (225Ac), designed to target carbonic anhydrase IX (CAIX), a cell surface antigen highly expressed in clear cell renal cell carcinoma (ccRCC) and other solid tumors. The drug acts as a targeted therapeutic radiopeptide by delivering cytotoxic alpha radiation directly to CAIX-expressing tumor cells. It is being developed by PeptiDream and its subsidiary PDRadiopharma for the treatment of ccRCC and potentially other cancers. Preclinical studies have shown robust antitumor effects in models such as colorectal cancer[4][8]. Clinical development has included phase 0 trials using the diagnostic analog 64Cu-PD-32766 for PET imaging in ccRCC patients, demonstrating high tumor uptake and favorable safety/pharmacokinetics profiles[1][3][7]. The therapeutic version with actinium-225 is expected to provide sufficient absorbed dose for effective tumor ablation based on dosimetry calculations from these studies.
