225Ac-YS5 is a CD46-targeted radioimmunoconjugate consisting of the humanized monoclonal antibody YS5 labeled with the alpha-emitting radioisotope actinium-225 (225Ac). CD46 is a cell surface protein that is highly expressed in various cancers, including metastatic castration-resistant prostate cancer (mCRPC) and its neuroendocrine or PSMA-low subtypes, while having limited expression in normal tissues. The YS5 antibody binds to a specific epitope on CD46, facilitating the delivery of high-energy alpha particles directly to tumor cells. These alpha particles cause lethal double-strand DNA breaks within a short range, minimizing damage to surrounding healthy tissue. Developed by researchers at the University of California, San Francisco (UCSF), 225Ac-YS5 has shown potent anti-tumor activity in preclinical models and is currently being evaluated in Phase 1 clinical trials as a targeted alpha therapy (TAT) for advanced prostate cancer.
