2A4 is a monoclonal antibody designed for the imaging and immunotherapy of AL amyloidosis. It specifically binds to a cryptic epitope found on amyloid fibrils, which is derived from a cleavage site of serum amyloid protein A (sAA) and contains a -Glu-Asp- amino acid pairing. Additionally, 2A4 demonstrates high-affinity binding to amyloid fibrils composed of immunoglobulin light chains. Studies have shown that radiolabeled 2A4 can specifically localize with human AL amyloid extracts, and treatment with 2A4 expedited the regression of ALκ amyloidomas in mice, likely mediated by macrophages and neutrophils.
