2F4 CAR-T is an experimental second-generation chimeric antigen receptor (CAR) T-cell therapy targeting the purinergic receptor 5 (P2RX5), specifically the full-length ancestral variant (P2RX5-FL). P2RX5 is a lineage-restricted surface antigen expressed in mature B-cells and various lymphoid and plasma cell malignancies, including B-cell lymphomas, multiple myeloma, and subsets of T-cell acute lymphoblastic leukemia. Due to specific genetic polymorphisms (rs3215407 and rs891746), the full-length protein is predominantly expressed in individuals of African ancestry, while most other populations express truncated, undetectable variants. The 2F4 CAR-T utilizes a single-chain variable fragment (scFv) derived from the 2F4 mouse monoclonal antibody to recognize P2RX5-FL. It is being developed as a genotype-guided immunotherapy to address relapsed/refractory disease, particularly in patients who have failed CD19- or BCMA-directed therapies.
