2h-AFL-PDT is a combination therapeutic protocol developed for the treatment of actinic keratosis, particularly on the face and scalp. The procedure begins with pretreatment using an Erbium:yttrium-aluminium-garnet (Er:YAG) ablative fractional laser (AFL), which creates microscopic treatment zones to enhance the skin's permeability. This is followed by the topical application of methyl aminolevulinate (MAL) cream, which is incubated for a shortened duration of 2 hours. During this time, MAL is preferentially taken up by dysplastic cells and converted into the endogenous photosensitizer protoporphyrin IX (PpIX). The area is then irradiated with a red light-emitting diode (LED) lamp, which excites the PpIX, leading to the production of reactive oxygen species and subsequent selective destruction of the actinic keratosis lesions. This protocol aims to maintain high efficacy while reducing the traditional 3-hour incubation time, thereby improving patient convenience and potentially reducing treatment-related pain.
