Overview

3,5-Diiodothyropropionic acid (DITPA) is a thyroid hormone analog investigated primarily for the treatment of congestive heart failure (CHF). It acts as a thyromimetic agent by mimicking thyroid hormone effects. In clinical studies including a phase II randomized controlled trial in patients with New York Heart Association class II to IV CHF, DITPA demonstrated beneficial hemodynamic effects such as increased cardiac index and decreased systemic vascular resistance. It also reduced serum cholesterol levels and body weight while suppressing thyroid-stimulating hormone without causing symptoms of hypothyroidism or thyrotoxicosis. Despite these metabolic and hemodynamic improvements, symptomatic benefit in CHF was not clearly established due to poor tolerability including fatigue and gastrointestinal complaints. DITPA has also been studied for its potential to induce alpha-MHC mRNA expression in heart cells.
