3-methylfentanyl is a highly potent synthetic opioid analgesic and an analog of fentanyl. It was first discovered in 1974 and is estimated to be between 400 and 6000 times more potent than morphine, depending on the specific isomer, with the cis(+) enantiomer being especially active. The drug acts as a mu-opioid receptor agonist, producing strong analgesic and sedative effects. It has no accepted medical use in the United States and is classified as a Schedule I controlled substance due to its high potential for abuse, lack of safety under medical supervision, and association with numerous overdose deaths. Illicitly produced, it has been sold as "synthetic heroin" or mixed with heroin[1][2][5][8].
