Overview

3-O-methylglucose (3-OMG) is a non-metabolizable glucose analogue that acts as a substrate for sodium-glucose cotransporter 1 (SGLT1) and glucose transporter 2 (GLUT2). It is widely used as an investigational research tool and physiological probe to study intestinal glucose absorption, gastric emptying, and glucose transport kinetics. In clinical studies, such as those conducted at the Royal Adelaide Hospital and the University of Adelaide, 3-OMG is administered as a sweet preload drink to evaluate its effects on plasma glucagon-like peptide-1 (GLP-1) release, gastric emptying, blood glucose, and appetite sensations in healthy volunteers and patients with type 2 diabetes mellitus. Preloads of 3-OMG have been shown to slow gastric emptying and stimulate GLP-1 and glucose-dependent insulinotropic polypeptide (GIP) secretion, thereby reducing postprandial glucose excursions. Additionally, radiolabeled or unlabeled 3-OMG is utilized in imaging studies, including positron emission tomography (PET) and chemical exchange saturation transfer (CEST) MRI, to evaluate glucose transport across the blood-brain barrier and detect malignant tumors.
