3D-cultured human placental mesenchymal stem cell-derived extracellular vesicles (hPMSC-EVs) are an investigational cell-free therapeutic being developed for the treatment of idiopathic pulmonary fibrosis (IPF). These extracellular vesicles, which include exosomes, are harvested from human placental mesenchymal stem cells grown in a three-dimensional (3D) culture environment, a method designed to better mimic the physiological niche and enhance the secretion of therapeutic factors compared to traditional 2D cultures. The vesicles contain a complex cargo of miRNAs, proteins, and lipids that exert anti-inflammatory and anti-fibrotic effects by modulating key signaling pathways involved in lung tissue scarring. Administered via a vibrating mesh nebulizer, hPMSC-EVs are delivered directly to the lower respiratory tract to promote tissue repair and stabilize lung function.
