3h-AFL-PDT-Dong-A University-actinic keratosis refers to a specific photodynamic therapy (PDT) protocol developed by researchers at Dong-A University for the treatment of actinic keratosis. This combination therapy utilizes an erbium:yttrium-aluminium-garnet (Er:YAG) ablative fractional laser (AFL) to create microscopic treatment zones, enhancing the skin penetration of the topical photosensitizer methyl aminolevulinate (MAL). Following a 3-hour incubation period—which has been clinically demonstrated to provide superior efficacy compared to shorter incubation times or conventional PDT—the lesions are irradiated with a red light-emitting diode (LED) lamp. The resulting photochemical reaction generates reactive oxygen species that selectively target and destroy dysplastic keratinocytes. This protocol has shown high complete response rates (91.7%) and significantly lower recurrence rates in clinical trials (NCT02248298) for facial and scalp actinic keratosis.
