Overview

**4-1BB-based CD19 CAR-T cells** are genetically engineered T cells expressing a chimeric antigen receptor (CAR) that targets CD19, a surface antigen found on B cells. The CAR construct specifically incorporates a 4-1BB (CD137) co-stimulatory domain, which plays a crucial role in enhancing T cell persistence, promoting a central memory phenotype, and reducing adverse events compared to alternatives such as CD28-based CAR-T cells. These cells are primarily developed for the treatment of B cell malignancies (such as relapsed/refractory B-cell non-Hodgkin lymphoma and acute lymphoblastic leukemia) and, more recently, are under clinical investigation for severe autoimmune diseases including systemic lupus erythematosus, myositis, systemic sclerosis, generalized myasthenia gravis, and pemphigus vulgaris. Clinical data show favorable safety (lower rates of severe cytokine release syndrome and neurotoxicity) and durable efficacy, related to robust depletion of pathogenic B cells and long-term remission in hematologic cancers and emerging autoimmune indications[1][2][6].
