Overview

A combination chemotherapy regimen consisting of 4'-epi-doxorubicin (also known as epirubicin), an anthracycline antibiotic and analog of doxorubicin, and cisplatin, a platinum-based chemotherapeutic agent. This combination has been investigated primarily in advanced solid tumors such as ovarian cancer, lung carcinoma, breast cancer, bladder carcinoma, renal cancer, and testicular cancer. **Mechanism of action:** - **Epirubicin (4'-epi-doxorubicin):** Intercalates into DNA and inhibits topoisomerase II activity, leading to inhibition of DNA replication and RNA synthesis; induces apoptosis in rapidly dividing cells. - **Cisplatin:** Forms DNA crosslinks (both interstrand and intrastrand), which inhibit DNA synthesis and function; also induces apoptosis through multiple mechanisms. The combination aims to exploit synergistic cytotoxic effects while maintaining manageable toxicity profiles[1][2]. Clinical studies have shown activity particularly in ovarian cancer and lung carcinoma with generally mild to moderate hematologic toxicity[2].
