4-Ethylguaiacol (4-EG) is a naturally occurring phenolic compound found in various foods, including coffee, corn, sesame, and green bell peppers. It is being investigated for its potent anti-inflammatory and neuroprotective properties, particularly in the context of autoimmune diseases like multiple sclerosis (MS). In preclinical models of MS, such as experimental autoimmune encephalomyelitis (EAE), 4-EG has demonstrated the ability to ameliorate disease severity by modulating neuroinflammation. Its mechanism of action involves the induction of Heme Oxygenase-1 (HO-1) expression in the central nervous system (CNS) and in myeloid cells like microglia and macrophages. Furthermore, 4-EG suppresses the activation of microglia, inhibits the differentiation of pathogenic Th1 and Th17 cells, and mitigates blood-brain barrier (BBB) disruption. Due to its ability to attenuate both peripheral and CNS-specific immune responses, it is considered a potential therapeutic candidate for neuroinflammatory disorders.
