Overview

4-hydroperoxyifosfamide (also known as 4-HOOI or HOO-IFOS) is a hydroperoxy-derivative of ifosfamide and a pre-activated prodrug of isophosphoramide mustard (IPM), which is the active alkylating metabolite of ifosfamide [3222]. Unlike ifosfamide, which requires hepatic cytochrome P450-mediated activation, 4-hydroperoxyifosfamide spontaneously decomposes in aqueous solution or intracellularly to release the active alkylating agent. This bypasses the need for hepatic activation and avoids the systemic release of toxic byproducts like chloroacetaldehyde and high levels of acrolein, thereby reducing associated central nervous system (CNS) and genitourinary (GU) toxicities [3222]. Developed by Dekk-Tec, 4-hydroperoxyifosfamide is lipophilic and capable of crossing the blood-brain barrier, making it a potential therapeutic candidate for primary and metastatic brain tumors, such as glioblastoma, as well as other advanced cancers.
