Overview

4B5 anti-idiotype vaccine + alum adjuvant is an experimental cancer vaccine combination developed by the University of Alabama at Birmingham (UAB) for the treatment of melanoma. The vaccine component, 4B5, is a human anti-idiotype monoclonal antibody (Ab2) that mimics the GD2 ganglioside antigen, which is highly expressed on the surface of melanoma cells. By acting as a surrogate for the GD2 antigen, the 4B5 antibody is designed to stimulate the patient's immune system to produce an active humoral and cellular response (Ab3 antibodies) against GD2-positive tumor cells. The alum adjuvant (aluminum salts) is added to the formulation to enhance the immunogenicity of the vaccine by promoting a more robust and sustained immune response. This therapeutic approach was evaluated in Phase I/II clinical trials (such as NCT00004184) to assess safety, toxicity, and immune response in patients with high-risk resected stage III or IV melanoma.
