**4D-150** is an investigational, single-dose intravitreal adeno-associated virus gene therapy developed by 4D Molecular Therapeutics for vascular retinal diseases. It uses the proprietary evolved retinotropic **R100** AAV vector to deliver a dual-transgene cassette encoding aflibercept and a microRNA-based RNA interference payload directed against VEGF-C. Locally expressed aflibercept neutralizes VEGF-A, VEGF-B, and placental growth factor, while the RNAi payload suppresses VEGF-C, thereby providing broad inhibition of four VEGF-family angiogenic mediators implicated in neovascular wet age-related macular degeneration and diabetic macular edema. The program is designed to provide multi-year retinal anti-VEGF activity after one routine intravitreal injection, reducing dependence on repeated bolus anti-VEGF injections. As of August 2026, 4D-150 is in Phase 3 development for wet age-related macular degeneration; diabetic macular edema remains in Phase 2 with Phase 3 initiation planned for the third quarter of 2026. It is not approved for marketing.
