Drug intelligence / Profile preview
The 4F peptide is an 18-amino acid synthetic amphipathic helical peptide designed to mimic the anti-inflammatory, antioxidant, and anti-atherogenic properties of apolipoprotein A-I (apoA-I), the primary protein component of high-density lipoprotein (HDL). It exists in two stereoisomeric forms: L-4F (APL180), synthesized from L-amino acids, and D-4F (APP018), synthesized from D-amino acids to resist proteolytic degradation and enable oral bioavailability. The peptide binds with high affinity to oxidized lipids (such as oxidized low-density lipoprotein and fatty acid hydroperoxides), sequestering them to prevent vascular inflammation and promote cholesterol efflux. Originally developed by Bruin Pharmaceuticals and licensed by Novartis, the 4F peptide has been clinically evaluated for cardiovascular diseases, including coronary artery disease and hypercholesterolemia, and has been investigated preclinically for age-related macular degeneration (AMD), Alzheimer's disease, and flavivirus infections (such as dengue and Zika).
