4G-CAR-GPC3 T cells are an autologous fourth-generation chimeric antigen receptor (CAR) T-cell therapy targeting glypican-3 (GPC3), a cell-surface protein highly expressed in hepatocellular carcinoma (HCC). Developed by Carsgen Therapeutics, these cells are engineered to co-express a transcription factor intended to enhance antitumor activity, persistence, and survival within the immunosuppressive solid tumor microenvironment. This fourth-generation construct represents an evolution from earlier second-generation GPC3-targeted CAR-Ts, aiming to overcome the limitations of cell therapy in solid tumors. In clinical settings, 4G-CAR-GPC3 T cells are being investigated for the treatment of advanced, heavily pretreated GPC3-positive HCC, often in combination with tyrosine kinase inhibitors (TKIs) such as sorafenib or regorafenib to potentially synergize and improve objective response rates.
