4SCAR-DLL3 T cells are an investigational fourth-generation chimeric antigen receptor (CAR) T-cell therapy targeting Delta-like ligand 3 (DLL3), a notch ligand selectively overexpressed in small cell lung cancer (SCLC) and other high-grade neuroendocrine carcinomas. The 4SCAR platform utilizes a lentiviral vector to engineer T cells with a CAR construct comprising a DLL3-specific single-chain variable fragment (scFv), tandem co-stimulatory domains (typically CD28 and 4-1BB), and the CD3ζ signaling domain. Uniquely, the 4SCAR design incorporates a safety mechanism, such as an inducible caspase-9 (iCasp9) suicide gene, which allows for the rapid elimination of the engineered T cells in the event of severe toxicity, such as cytokine release syndrome. This therapy is designed to provide a potent and controlled immune response against DLL3-positive tumor cells while minimizing off-target effects on healthy tissues.
