4SCAR-T cells for lung cancer are an investigational cell therapy developed by the Shenzhen Geno-Immune Medical Institute (GIMI). This approach utilizes fourth-generation chimeric antigen receptor (CAR) T cells, which are engineered to incorporate multiple costimulatory domains and a safety switch, such as inducible caspase-9 (iCasp9), to enhance anti-tumor efficacy and allow for the controlled elimination of the cells in the event of severe toxicity. The therapy targets various lung cancer-associated antigens, including EGFR, MUC1, CEA, and Mesothelin, to treat patients with advanced or relapsed non-small cell lung cancer (NSCLC) or small cell lung cancer (SCLC). The treatment protocol often involves the use of artificial antigen-presenting cells (aAPCs) to expand antigen-specific T cells (sometimes referred to as EIEs or ex vivo expanded cells) before re-infusion into the patient, aiming to overcome the immunosuppressive tumor microenvironment characteristic of solid tumors.
