4SCAR T cells is a fourth-generation chimeric antigen receptor (CAR) T-cell therapy developed by the Shenzhen Geno-Immune Medical Institute for the treatment of autoimmune diseases. The therapy utilizes a lentiviral vector to engineer T cells to express CARs targeting multiple B-cell surface antigens, including CD19, BCMA (B-cell maturation antigen), CD138, and BAFF-R. This multi-target approach is designed to achieve comprehensive depletion of pathogenic B cells and plasma cells involved in autoantibody production. A defining feature of the 4SCAR platform is the incorporation of an inducible caspase 9 (iCasp9) safety switch, which allows for the controlled elimination of the CAR-T cells in the event of severe toxicity. In clinical trials, such as NCT05459870, the therapy is often evaluated in combination with fetal mesenchymal stem cells (MSCs) to modulate the immune response and improve safety. The therapy has demonstrated a high safety profile in early-phase trials, with minimal cytokine release syndrome or neurotoxicity.
