4SCAR20 + 4SCAR22 + 4SCAR70 + 4SCAR-PSMA + 4SCAR13 + 4SCAR79b + 4SCAR-GD2 is an investigational multi-target chimeric antigen receptor (CAR) T-cell therapy developed by the Shenzhen Geno-Immune Medical Institute (GIMI). Part of the 4SCAR2.0 regimen, this therapy is designed to treat relapsed or refractory B-cell lymphomas by simultaneously or sequentially targeting a broad array of tumor-associated antigens, including CD20, CD22, CD70, PSMA, CD123, CD79b, and GD2. This multi-pronged approach aims to overcome antigen escape, a common cause of relapse in single-target CAR-T therapies. The T cells are genetically modified using lentiviral vectors to express fourth-generation CAR constructs that include CD28, CD27, and CD3ζ signaling domains. For enhanced safety, the cells incorporate an inducible caspase-9 (iCasp9) suicide gene, which can be activated to eliminate the CAR-T cells in the event of severe toxicity. The therapy is currently being evaluated in Phase I/II clinical trials as a primary, booster, and consolidation treatment strategy.
