4SCAR38 is an investigational 4th-generation chimeric antigen receptor (CAR) T-cell therapy targeting the CD38 antigen, often expressed in hematological malignancies. Developed using the 4SCAR (4th-generation Safety CAR) platform, the construct features a chimeric receptor comprising an anti-CD38 single-chain variable fragment (scFv) linked to a tandem array of three costimulatory domains—CD28, CD137 (4-1BB), and CD27—designed to enhance T-cell expansion, persistence, and anti-tumor efficacy. To mitigate potential severe toxicities such as cytokine release syndrome, the design incorporates an inducible caspase 9 (iC9) safety switch, which can be activated by a small-molecule dimerizer to rapidly induce apoptosis in the CAR-T cells. 4SCAR38 is primarily being evaluated for B-cell malignancies and multiple myeloma, frequently in combination with other CAR-T products targeting antigens like CD19 or CD22 to prevent antigen-negative relapse.
