Overview

This is a four-drug chemotherapy combination consisting of 5-fluorouracil (5-FU), folinic acid (leucovorin), gemcitabine, and oxaliplatin. Each component has a distinct mechanism of action: - **5-fluorouracil** is an antimetabolite that inhibits thymidylate synthase, disrupting DNA synthesis. - **Folinic acid** enhances the binding of 5-FU to thymidylate synthase, increasing its cytotoxicity. - **Gemcitabine** is a nucleoside analog that incorporates into DNA and inhibits DNA synthesis by blocking ribonucleotide reductase. - **Oxaliplatin** is a platinum-based agent that forms crosslinks in DNA, leading to apoptosis. This combination has been investigated primarily for advanced or metastatic gastrointestinal cancers such as gastric cancer and esophageal cancer. The regimen aims to leverage synergistic effects between these agents for improved anti-tumor activity[1][3]. It remains investigational and has been studied in phase II clinical trials.
