5-fluorouracil + mitomycin is a combination chemotherapy regimen primarily used as a radiosensitizer in the definitive treatment of squamous cell carcinoma of the anal canal. 5-fluorouracil (5-FU) is a pyrimidine analog antimetabolite that inhibits thymidylate synthase, thereby disrupting DNA synthesis. Mitomycin (also known as mitomycin C) is an antitumor antibiotic that acts as a potent DNA cross-linking agent, inducing DNA damage and inhibiting replication. In the clinical trial led by Jordan Kharofa at the University of Cincinnati (NCT03397459), this regimen is administered concurrently with Intensity Modulated Proton Therapy (IMPT) to evaluate the feasibility of reducing treatment-related toxicities in patients with stage T2-4 anal cancer compared to traditional photon-based radiation therapy.
