5-grass pollen extract is an allergen immunotherapy (AIT) product developed by Probelte Pharma, containing a standardized mixture of extracts from five common grasses: *Dactylis glomerata* (Orchard grass), *Festuca pratensis* (Meadow fescue), *Lolium perenne* (Perennial ryegrass), *Phleum pratense* (Timothy grass), and *Poa pratensis* (Kentucky bluegrass). The therapy is indicated for the treatment of allergic rhinitis, allergic conjunctivitis, and controlled allergic asthma in patients with IgE-mediated hypersensitivity to grass pollens. Its mechanism of action involves the induction of immune tolerance by shifting the immune response from a Th2-dominated profile to a Th1-dominated profile, stimulating the expansion of regulatory T cells (Tregs), and increasing the production of allergen-specific IgG4 antibodies. These IgG4 antibodies act as blocking antibodies that compete with IgE for allergen binding, thereby preventing mast cell degranulation and the subsequent release of inflammatory mediators. The product is available in both subcutaneous (often adsorbed on aluminum hydroxide) and sublingual formulations.
