5-Hydroxytryptophan (5-HTP) is a small molecule serotonin precursor being investigated by the University of Alberta for the treatment of spinal cord injury (SCI). In clinical study NCT04520178, 5-HTP is co-administered with carbidopa, a peripheral decarboxylase inhibitor, to facilitate its crossing of the blood-brain barrier and minimize peripheral side effects. Once in the central nervous system, 5-HTP is converted into serotonin (5-HT) by the enzyme aromatic L-amino acid decarboxylase (AADC). The increased serotonin levels are intended to activate 5-HT2 receptors on spinal motoneurons, thereby modulating neural excitability and potentially improving motor function in patients with subacute or chronic spinal cord injuries. This pharmacological approach aims to compensate for the loss of descending serotonergic input typically observed following spinal cord trauma.
