5MPN is a small molecule inhibitor of 6-phosphofructo-2-kinase/fructose-2,6-biphosphatase 4 (PFKFB4), an enzyme that plays a critical role in cancer metabolism by diverting glycolytic flux into the pentose phosphate pathway (PPP). By inhibiting PFKFB4, 5MPN disrupts the production of NADPH, which is essential for maintaining antioxidant defenses and supporting mitochondrial oxidative phosphorylation (OXPHOS). In the context of acute myeloid leukemia (AML), 5MPN treatment leads to oxidative stress, mitochondrial dysfunction, and the induction of apoptosis in leukemic cells. Preclinical evidence indicates that 5MPN can significantly prolong survival in AML models, particularly when used in combination with standard induction chemotherapy, by targeting metabolic vulnerabilities and overcoming chemoresistance.
