64Cu-DOTA-pembrolizumab is an investigational radiopharmaceutical used as an immuno-PET (Positron Emission Tomography) imaging agent. It consists of the PD-1-targeting monoclonal antibody pembrolizumab conjugated to the chelator DOTA (1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid) and labeled with the positron-emitting radioisotope Copper-64 (64Cu). The primary mechanism of action involves the specific binding of the pembrolizumab component to the Programmed Cell Death Protein 1 (PD-1) receptor on T cells and within the tumor microenvironment. By utilizing the radioactive decay of 64Cu, clinicians can non-invasively visualize and quantify the spatial distribution and density of PD-1 expression throughout the body. This imaging tool is primarily developed to identify patients most likely to respond to PD-1/PD-L1 checkpoint inhibitor therapies and to monitor changes in receptor expression during treatment.
