64Cu-DOTA-trastuzumab is an investigational radiopharmaceutical diagnostic agent used for positron emission tomography (PET) imaging. It consists of the monoclonal antibody trastuzumab, which targets the human epidermal growth factor receptor 2 (HER2), conjugated to the positron-emitting radioisotope copper-64 (64Cu) via the DOTA (1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid) chelator. The agent is designed to non-invasively visualize and quantify HER2 expression in tumors, helping to identify HER2-positive lesions in patients with breast or gastric cancer. It is particularly useful for detecting metastases, including those in the brain, and for predicting patient response to HER2-targeted therapies such as ado-trastuzumab emtansine (T-DM1) or trastuzumab deruxtecan (T-DXd) by assessing the biodistribution and tumor uptake of the antibody.
