64Cu-GRIP B is a diagnostic radiopharmaceutical and peptide conjugate radionuclide designed for positron emission tomography (PET) imaging. It specifically targets extracellular granzyme B, an enzyme secreted by activated immune cells in the tumor microenvironment, particularly following immunotherapy such as immune checkpoint inhibitors or CAR-T cell therapy. The drug consists of a peptide substrate optimized for cleavage by human granzyme B, conjugated to DOTA and radiolabeled with copper-64. Upon systemic administration, the inactive pro-form of the probe is cleaved by extracellular granzyme B at sites of immune activation, releasing a radiolabeled antimicrobial peptide that binds to nearby cell membranes—enabling visualization of active anti-tumor immunity via PET imaging. This mechanism allows noninvasive assessment of immune response within tumors and may help predict or monitor responses to immunomodulatory therapies in cancer patients[1][2][5][6].
