64Cu-RYM2 is a first-in-class radiopharmaceutical imaging agent developed by the Washington University School of Medicine for the detection and risk stratification of abdominal aortic aneurysms (AAA). The agent consists of RYM2, a macrocyclic hydroxamate-based broad-spectrum matrix metalloproteinase (MMP) inhibitor, which is functionalized with a NOTA chelator and radiolabeled with the positron-emitting isotope copper-64 (64Cu). 64Cu-RYM2 specifically targets activated MMPs, including MMP-12, which play a critical role in the degradation of the extracellular matrix and the subsequent pathogenesis and rupture of aneurysms. By utilizing PET/CT imaging, this tracer allows for the non-invasive visualization of MMP activity in the aortic wall, potentially providing a more biological assessment of aneurysm stability compared to traditional diameter-based measurements.
