67Ga-pentixafor is a radiopharmaceutical diagnostic agent used for Single Photon Emission Computed Tomography (SPECT) imaging. It consists of the cyclic pentapeptide Pentixafor, which has high affinity and selectivity for the C-X-C chemokine receptor type 4 (CXCR4), chelated with the radioisotope Gallium-67. CXCR4 is a chemokine receptor frequently overexpressed in a wide range of cancers, including hematological malignancies like multiple myeloma and various solid tumors, where it plays a role in tumor growth, progression, and metastasis. By binding to CXCR4, 67Ga-pentixafor allows for the non-invasive visualization and quantification of receptor expression. Compared to the more common PET agent 68Ga-pentixafor, the 67Ga version utilizes the longer half-life of Gallium-67 (78.3 hours), facilitating delayed imaging and potential use in centers without PET capabilities or for specific dosimetric calculations prior to CXCR4-targeted therapy.
